Die ersten Abschnitte der Strecke wurden im Jahre 1866 eröffnet, um Gemeinden von Holstein mit den Ostseehäfen zu verbinden. Die ersten Gleise erreichten Lübeck jedoch erst im Jahr 1873, nach der Gründung des Deutschen Kaiserreiches.

Als die Stecke vollständig eröffnet war, wurde sie von mehreren Betrieben unterhalten. Es wurden vor allem Fahrgäste befördert, die die wunderschönen Orte der Region besuchten. In den 1930er Jahren lagen die Reisezeiten durchschnittlich bei 150 Minuten, was der D-Zug auf 95 verkürzte. Heute können Regionalzüge die Strecke in etwas über einer Stunde bewältigen.

Anders als viele andere Strecken in Deutschland ist die Strecke Kiel-Lübeck bis heute nicht elektrifiziert und zum Großteil eingleisig, was bedeutet, dass sie eine Oase für Dieselzüge ist und an mehreren Bahnhöfen entlang der Strecke Ausweichgleise besitzt.
